Application Cost:
In Person: Rs.500/- (General); Rs.100/- (Special Category) and
SC / ST candidates are exempted from payment of cost of application
and Prospectus. They can obtain the application on written requisition
duly enclosing two sets of attested xerox copy of the Community
Certificate.
By Post: Rs.500/- (General); Rs.100/- (Special Category - Physically
Handicapped / Children of Freedom Fighters / Children of Ex-Servicemen
/ Eminent Sports persons) and SC / ST candidates are exempted
from payment of cost of application and Prospectus - duly enclosing
the postal stamp to the value of Rs.60/- (Rupees Sixty only)
for ordinary post along with the DD.
Mode of Payment (DD / Cash):
Demand Draft in favour of "The Secretary, Selection Committee,
Kilpauk, Chennai - 600 010" payable at Chennai from any Nationalised
bank. The name and address of the candidate should be written
in the reverse of the demand draft.
Application Forms:
- In Person: Application
forms can be obtained with a written requisition to the
Deans / Principal of the below Government Medical Institutions
duly enclosing a crossed Demand Draft from the following
Colleges:
> Madras Medical College, Chennai
> Stanley Medical College, Chennai
> Madurai Medical College, Madurai
> Thanjavur Medical College, Thanjavur
> Kilpauk Medical College, Chennai
> Chengalpattu Medical College, Chengalpattu
> Tirunelveli Medical College, Tirunelveli
> Coimbatore Medical College, Coimbatore
> Mohan Kumaramangalam Govt. Medical College, Salem
> K.A.P.Viswanatham Government Medical College, Trichy
> Thoothukudi Govt. Medical College, Thoothukudi
> Kanyakumari Govt. Medical College, Asaripallam, Nagarcoil
> Vellore Govt. Medical College, Vellore
> Theni Govt. Medical College, Theni
> Tamil Nadu Govt. Dental College, Chennai
- By Post: Applications
can also be obtained by addressing the Dean / Principal
of Government Medical / Dental College, duly enclosing the
postal stamp to the value of Rs.60/- (Rupees Sixty only)
for ordinary post along with the required Demand Draft (Rs.500/-
or Rs.100/- as the case may be). The SC/ST candidates need
not enclose Demand draft for Rs.500/-.

and send the application along with the crossed Demand Draft.
